- Medicaid Service Coordination – This service provides linkage to the individuals and their families. It ensures access to needed services such as entitlements, social, clinical, vocational, recreational and other support services.
- Non-Medicaid Service Coordination – We are able to provide service coordination to those individuals and their families that are not eligible for Medicaid. These individuals have access to services such as respite, advocacy, home health aids, and vocational, recreational and other support services.
- Day Habilitation Program – A Day Habilitation Program whose primary goal is to base activities upon the wants, desires, dreams, interests, and abilities of the participants. The program focuses on areas that foster personal growth, develop vocational skills, enhance social and communication abilities, and provide each individual with goal oriented activities; social interaction, behavior coping skills, enhancement of Daily Living Skills; travel training, community inclusion, job training through volunteerism; Increased independence and productivity and understanding of community through active participation in community events.
- OPTS “Transitions” After-School & Saturday Recreation Program – “Transitions” offers Quality and Rewarding experiences for individuals in person centered age-appropriate activities in a safe, structured, positive environment. The major functions provided by our After-School/Saturday Recreation Program are to provide supervision, offer enriching experiences, positive social interaction, and to improve academic achievement. In addition Human First offers behavior management training and community inclusion in a highly therapeutic environment geared to decreasing challenging behaviors and increasing positive social interaction. In addition to After-School and Saturdays this program covers school closing dates including Winter Break, Mid-Winter Break and Spring Break.
- Crisis Intervention – This program is geared towards families in crisis due to intense behavioral problems. Our program has a comprehensive aim and approach to eliminate any related issues to relieve the crisis situation. Our program goal is for an individual is for them to fit into mainstream programs with a smooth transition. A behavioral psychologist provides an assessment and a plan of intervention. Each individual receives 15 hours of 1 to 1 service per month.
- Community Habilitation – This program is for individuals who live at home and would benefit from specialized training targeting the enhancement of daily living activities, communication and socialization and community awareness skills.
- Family Education & Training – A program designed to assist by educating parents of all available benefits and entitlements and in addition to learn to develop their advocacy skills. Human First offers training to families with a curriculum that includes, but not limited to, family life, the educational system, health and personal care, OMRDD and DOH services, legal and financial issues.
- Reimbursement for Goods and Services – A program to provide families with goods and services for developmentally disabled individuals who are without Medicaid. This program pays for expenses related to the daily living needs of persons with developmental disabilities.
