Medicaid Service Coordination This service provides linkage to the individuals and their families. It ensures access to needed services such as entitlements, social, clinical, vocational, recreational and other support services.
QUALIFICATIONS:
- Bachelor's degree in Social work from accredited College or University
- Bachelor’s degree in a related field (Psychology/Special Education)
- Associate's degree in a related field with at least one year experience in the MR/DD
REQUISITE SKILLS:
- Excellent organization, writing and public relation skills.
- Willingness to travel as needed to other program sites and consumer’s homes. Drivers’ license and car a must.
JOB DUTIES:
- Evaluate, plan, prepare and implement individualized service plans (ISP’s)
- Ensure that ISP’s are current and up-to-date and fully explained to and understood by the consumer/family.
- Actively participate in consumer reviews and service planning meetings.

Residential Habilitation Trainer
QUALIFICATIONS:
- High School Diploma and/or some college credits required. 1 year experience
working with individuals with developmental disabilities strongly preferred.
DESCRIPTION:
- Provide, assist and deliver habilitation supports and services that promote
choice to the individuals served by Human First in achieving our agency mission
under the guidelines of OMRDD including but not limited to:
- Creating opportunities for, and
encouraging, individuals to develop skills in recreation/leisure/hobbies, home
management, social/interpersonal and other areas while developing relationships
in the community.

Assisting in consumer personal hygiene, grooming, feeding, care for consumer
belongings, room cleaning, and other daily living skills.
